DO I NEED A DOCTOR’S REFERRAL?

Check with your insurance. Most insurances require a Doctor's referral in order to attend physical therapy.

We accept all major insurances such as: BCBS, Moda, Pacific Source, Providence, Atrio, Cigna, Healthnet and more. At this time we are not in contract with OHP so we are not able to take them as a primary insurance.

WHAT IS YOUR NEW PATIENT PROCEDURE?

When you arrive for your Initial appointment please bring your ID and insurance information. We will also have some paperwork for you to fill out so please try to arrive about 15 minutes early.
